Person-Centered Support Built For Outcomes

Homemaker & Personal Care

Homemaker & Personal Care, often called HPC, supports a person in becoming more independent while meeting their daily living needs. HPC advances the individual's independence within their home and community, helping them meet day-to-day living needs. Examples of support include:

Self-advocacy and self-direction
Expressing personal preferences, asserting rights, and making increasingly independent choices.
Daily living skills
Meal preparation, personal care, and self-administration of medication.
Therapeutic & behavioral support
Reinforcing physical, occupational, and speech therapy goals and appropriate expressions of emotion.
Medical & health care
Routine medication administration and attentive support for ongoing medical needs.
Emergency response training
Preparedness planning and training on equipment and technologies.
Community access
Connecting to community resources and activities that build independence.
Personal finances
Assistance when provided alongside other homemaker/personal care supports.
